- Course Audit Request Use this form to request to audit a course
- Pass/No Pass Grade Request Use this form to request to receive a Pass/No Pass grade for a course
- Time Conflict Request Use this form to request a time conflict where the meeting times classes overlap
- High School Dual Enrollment Request (Non-CCAP Courses) Use this form if you are currently enrolled in high school and want to take a course at WCC
- High School Signatories Form (Designated School Officials) Principals use this form to request to designate of authorization of signatories for students who want to enroll in dual enrollment course(s)
- Prerequisite Challenge Use this form to request to challenge prerequisite course.
- Student Records Change Request Use this form to request change your academic goals (add or remove programs)
- Official Transcript Request Please go to National Student Clearinghouse at https://tsorder.studentclearinghouse.org . OR Use this form to request a hard copy of your official transcripts
- Duplicate Diploma Request
- Credit for Prior Learning Eligible students may receive credit for knowledge and skills obtained outside of the classroom. Use this form to request credit for prior learning
- FERPA Block Request Use this form to prevent the release of directory information, as defined in the College Catalogs, to third parties.
- FERPA Authorization for Release Use this form to allow district officials to discuss specific, non-directory information about your educational record to third parties
- Preferred Name Change Form Use this form to request a change of personal information
- Loss of Enrollment Priority CCPG Appeal 07.15.19 Use this form to appeal your enrollment priority and/or loss of California College Promise Grant
The following forms require Counselor approval prior to processing:
- Student Petition Use this form to petition to repeat a course, carry a unit overload (19 units or above), Readmission to WCC after academic dismissal, academic renewal, and other circumstances (discuss with a counselor)
- Third Attempt Course Repeat Request Use this form to request a third attempt to repeat a course
- Academic Renewal Use this form to request an Academic Renewal. Academic Renewal removes “D”, “F”, and "FW" grades from your Grade Point Average (GPA) calculations (see form for the different rules that apply)
- Residency Reclassification Application Use this form to request a review of your residency status
- CA Nonresident (AB540) Tuition Exemption Request Use this form to request exemption from nonresident tuition charged to nonresident students
