DSPS Application Forms
- DSPS Application (Only enrolled students, who are logged into the WCC Student Portal, can apply to DSPS through AIM).
- DSPS Disability Verification and Consent (Verification of disability is required to be served by DSPS. Other acceptable verifications include: medical records, IEPs, 504s, or other official documentation that includes the diagnosis of disability.)
Other Forms
- Accommodations Request Form (It is the student’s responsibility to request their accommodations each semester.)
- Alternate Media Request Form (Students must request Alt Media, and provide proof of purchase or possession of textbooks/materials, before requests can be fulfilled.)
- Test Proctoring Appointment Request Form (Students must request/schedule appointments for DSPS-proctored exams, at least 3 business days in advance.)

- Consent for Disclosure to Parent(s) or Guardian(s)
