DSPS Application Forms
- DSPS Application (Only enrolled students, who are logged into the WCC Student Portal, can apply to DSPS through AIM).
- DSPS Disability Verification and Consent (Verification of disability is required to be served by DSPS. Other acceptable verifications include: medical records, IEPs, 504s, or other official documentation that includes the diagnosis of disability.)
Other Forms and Requests
- Accommodations Request for the Semester (It is the student’s responsibility to request their accommodations through AIM each semester.)
- Alternate Media Requests (Students must request Alt Media, such as Kurzweil audiobooks and Jamworks recording and transcription, through AIM. Proof of purchase or possession of textbooks/materials is required before Kurzweil audiobook requests can be fulfilled.)
- DSPS-Proctored Exam Appointment Requests (Students must schedule appointments in AIM for DSPS-proctored exams, at least 3 business days in advance.)

- Consent for Release of Information
- Consent for Disclosure to Parent(s) or Guardian(s)
